Pacific Accountable Care, LLC (PAC) was approved by the Centers for Medicare & Medicaid Services to begin services on January 1, 2017. We are dedicated to working in a consistent and cohesive manner to improve the care of Medicare Beneficiaries and lower the growth in overall health care expenditures.  Pacific Accountable Care is composed of tremendously gifted providers who voluntarily agreed to work together to provide high standards of healthcare through a philosophy of continuous quality improvements aimed at achieving improved care and outcomes in a consistent manner.
 
Pacific Accountable Care will guide and foster a culture of transparency through continuous improvement, communication and teaching opportunities throughout the continuum of care.  Our mission is simple: proving that providing the right care; at the right time is the best form of coordinated and high quality of care.
